2010 Summer Program Registration Forms:

Step 1: Download or print the registration form by clicking on the appropriate link below.

Summer Camp Registration: Download/print BOTH of the following forms. (Please note: The Health Form is required at the time of registration. You may update information at any point prior to the start of camp.)

Step 2: Attach appropriate payment/deposit.

With the exception of 1,2,3…Akita! which requests payment in full, all registrations require a minimum $75 non-refundable deposit. Make checks payable to First Community Church, with your child’s name in the Memo line.

Step 3: Return completed registrations and payment to the North Campus of First Community Church.

If you are dropping off forms after business hours or on the weekend, there is a locked white after-hours mail box, located near the new wing entrance in front of the handicap parking spots.
